Acute Optic Neuritis Case Managed at Veda Eye Care, Sola

Patient Presentation

A 28-year-old female reported sudden reduction of vision in her left eye for 3 days, along with pain on eye movement. There was no history of trauma, fever, recent infection, or systemic illness. She had no prior neurological complaints.


Clinical Findings

Visual Acuity: Right eye 6/6, Left eye 6/60

Color Vision: Reduced in the left eye on Ishihara testing

RAPD: Present in the left eye

Anterior Segment: Normal in both eyes

Fundus Examination: Mild disc hyperemia with blurred margins in the left eye


Investigations

MRI Brain and Orbit: Swollen optic nerve on the left side

Blood Tests: Within normal limits


Diagnosis

Based on symptoms, clinical examination, and MRI findings, the patient was diagnosed with acute unilateral optic neuritis.


Treatment Given

Intravenous methylprednisolone 1 g per day for 3 days

Followed by oral prednisolone 1 mg per kg per day, tapered over 11 days

Outcome

At 4 weeks follow-up, vision in the left eye improved to 6/9. Color vision and contrast sensitivity showed marked recovery. There was no recurrence during 6 months of follow-up.
